The Yolo County Children’s Alliance is looking for a dynamic Community Liaison for the Help Me Grow (HMG) program. The Community Liaison will be linking families and children to existing resources in the community and providing the necessary care coordination to ensure the families are connected to services. She/he will conduct developmental screenings; meet with parents; handle telephone calls; enter data; search the resource database; conduct follow-up care coordination; collaborate with community agencies and healthcare providers via outreach; and participates in other activities job related activities.
