Yolo County Children’s Alliance is building a brand-new Community Advisory Board for our We Prosper Together – Community Health Worker pilot, and we’re looking for bold, committed leaders to help shape its future. If you are rooted in community, healthcare, government, or education—and want to use your voice to influence a project designed to uplift young mothers and young parents—this is your opportunity to make a meaningful impact.

Project Summary
Valley Vision is funding a Community Health Worker (CHW) pilot through We Prosper Together that empowers young mothers—particularly those from BIPOC and rural communities in Yolo County—with the skills, support, and career pathways needed to achieve long-term economic mobility. Participants complete CHW certificate training through Sacramento City College and gain hands-on experience with local healthcare systems, building both confidence and real-world expertise. Training includes:
- Patient navigation
- Health education
- Community outreach and prevention
- Climate-related health impacts and resilience
This pilot is designed to open doors, strengthen families, and build workforce pathways that reflect the real needs of our community. The commitment for CAB members includes monthly meetings from January 2026 through September 2026, along with participation in three community forums.
Mission of the Community Advisory Board
To ensure the We Prosper Together program stays community-driven, equitable, and grounded in the lived experiences of young mothers, BIPOC communities, and rural residents across Yolo County.
What the CAB Will Influence

Your voice will help shape key elements of the program, including:
- Curriculum content and delivery
- Support services and resource allocation
- Staff training and cultural competency expectations
- Recruitment and outreach approaches
- Community engagement events and strategies
- Evaluation metrics and how we define “success”
- Budget priorities within flexible funding areas
- Partnerships with employers and community partners
- Program policies and participant requirements
- Recommendations for sustainability and scaling

About YCCA
Yolo County Children’s Alliance is a 501(c)(3) that ensures everyone in Yolo County has access to the resources they need to build meaningful ties to their communities. To accomplish this, YCCA has expanded on our foundational role as the designated Child Abuse Prevention Council of Yolo County. Today, we serve as an inter-agency coordinator, building networks of support throughout Yolo County to deliver programs funded by numerous community and government grants, as well as individual donations. Our work focuses on three family-strengthening pillars: Family Support, Parent Education, and Community Collaboration.
